    4 L型細菌的耐藥性與醫院感染
      細菌變成L型后與其原菌株不同,故臨床治療用藥時,不僅要考慮細菌的藥敏,也要考慮L型的藥敏。一般認為細菌變為L型后已經失去了細胞壁,而β-內酰胺抗生素(青霉素類抗生素)的作用主要是抑制肽聚糖的合成,所以對細菌L型不起作用,而對作用于核酸與蛋白質合成的藥物如紅霉素、氯霉素敏感。雖然臨床分離出的細菌L型有不同程度的細胞壁缺陷,且常不穩定,但從許多病例分離出的細菌L型對作用于細胞壁的先鋒霉素仍顯敏感。先鋒霉素雖也屬β-內酰胺類抗生素,但它對β-內酰胺酶的作用穩定,而且先鋒霉素也能作用于微生物的細胞膜,故臨床選用先鋒霉素大多有效。
      臨床病例中分離到的細菌型和L型對藥物敏感性不同,因此在臨床治療時應同時考慮二者的藥物敏感情況,采取聯合用藥方案,主張應根據所分離到的細菌及L型的藥物敏感試驗結果來決定用藥,主張作用于細胞壁和細胞質的抗生素聯合應用,以達到殺菌并阻止其變成細菌L型。
      穩定的細菌L型可長期潛伏在機體內,使機體產生特異性抗體,使患者形成慢性病灶,反復發作,如腎炎、風濕性關節炎等。細菌L型能通過胎盤垂直感染胎兒,細菌L型的原生質體同中等大小的病毒相似。細菌L型的致病性,不僅返祖后具存,某些L型細菌本身也能致病。L型細菌可能是某些傳染病(如鼠疫、結核病和霍亂等)病原體的一種保存形式,在臨床活動中該菌也是一類值得注意的耐藥性微生物。

      Dorozkova[18,19]發現結核的復發與分枝結核桿菌的L型感染有關。他對2412例結核病例進行了研究,發現214例患者分離出了結核菌L型。進一步研究顯示:細菌L型引起的結核病容易復發,表明分離出不穩定的L型是一個重要的預測信號,顯示了結核病復發的高度危險性,Glanov調查表明:伴有細菌L型的結核患者癥狀較重,易于進展,且預后不良。慢性結核病灶內,結核菌以L型為主,且形態多樣,有圓球體、巨形體和長絲體等,其形態明顯不同于結核菌。由于結核菌L型細胞壁中的抗原成分丟失或減少,其致病性相對減弱,常缺乏明顯的臨床癥狀和體征,造成臨床誤診。結核菌L型也是結核菌抵抗外界環境和賴以生存的重要形式,細菌L型可在體內長期存活,生長、繁殖,導致結核病緩慢地進展,L型細菌又可大量繁殖,引起L型敗血癥或回復為結核菌,導致結核病惡化進展。結核菌L型也是重要的傳染源之一,故應高度重視結核菌L型的檢出。
      中耳炎患者分泌物細菌培養大多陰性,培養不出細菌,但該病有強烈的感染指征[20]。Ataoglu采用心腦肉湯和特殊的高滲硫乙醇酸鈉肉湯對40例中耳炎患者分泌物進行培養,其中6例在硫乙醇酸鈉法有L型細菌生長,而常規培養無細菌生長,因此他認為:經久不愈的慢性中耳炎與細菌L型感染有重要的關系。
      由于抗生素的廣泛應用等因素,使細菌L型感染增多,細菌L型感染機會的廣泛存在,以往由于細菌L型培養未能廣泛開展而未被發現。引起感染的L型病原菌以金黃色葡萄球菌為多,其次為化膿性鏈球菌、表皮葡萄球菌、鼠傷寒沙門氏菌、銅綠假單胞菌、大腸埃希氏菌、枸櫞酸桿菌等。L型引起的醫院感染多發生于慢性疾病、體質虛弱的患者。患者在非感染性疾病或原發病的基礎上繼發新的感染,病情較重。尤其金黃色葡萄球 菌與鼠傷寒沙門氏菌感染往往病情危重。

      細菌L型為細胞壁缺損的變異菌,抗生素廣泛應用是誘導細菌L型形成的重要因素。醫院感染開始可為普通細菌感染,經抗生素治療后形成L型,也可開始即為L型感染,造成細菌L型醫院感染的因素有:機體抵抗力降低;大量應用廣譜抗生素;感染性疾病沒有及時治療;慢性遷延性疾病。引起醫院感染的細菌L型抗藥性強,治療困難,根據藥物敏感試驗結果,合理應用抗生素,同時應用免疫抑制劑及輸血或血漿,加強支持療法。

      細菌L型培養的開展與改進,使感染病例培養陽性率明顯提高。因此,醫院感染科應廣泛開展細菌L型培養以提高細菌L型感染的診斷與治療。
